The TMS Athletic Packet must be completed online through Rank One Sport. All forms must be succesfully submitted prior to participation!

Important: If your child attends a school other than Taos High School then the following information needs to be submitted to the appropriate head coach: Name, Date of Birth, Student ID, Address, Emergency Contact and Contact Phone Number
